Registration/Waiver Form

 To obtain a registration/waiver form, please click on the race date below. The form will be downloaded in Adobe PDF format. If you do not have Adobe Reader, then click on the "Get Adobe Reader" icon, to download and install the software needed to display and print the form. The application / waiver form should be signed and completed by at least one team member. Additional team members will sign a waiver on day of race. Return the form with your check to:
